First: Diagnosis of infertility

1) Diagnosis of infertility in women
Basic tests
1.    Medical history and clinical examination
To determine cycle regularity, hormone symptoms, and the presence of pain or inflammation.
2.    Hormone tests often include:
•    FSH – LH
•    AMH (ovarian reserve)
•    Prolactin
•    TSH
•    Progesterone (to assess ovulation)
3.    Ultrasound
To examine the ovaries and endometrium and detect cysts or fibroids.
4.    Fallopian tube examination (HSG)
To ensure there is no blockage.
5.    Laparoscopy (when necessary)
To diagnose endometriosis and adhesions.

2) Diagnosis of male infertility

Basic tests
1.    Semen analysis
The most important test, which evaluates:
•    Count
•    Motility
•    Morphology
•    Viscosity
2.    Hormone tests
•    Testosterone
•    FSH, LH
•    Prolactin
3.    Testicular ultrasound
To detect varicocele, inflammation, or obstruction.
4.    Additional tests when needed
•    Sperm DNA fragmentation test
•    Testicular biopsy in cases of azoospermia

Second: Treatment of infertility

1) Treatment of infertility in women

Treatment depends on the cause, and the most prominent options are:

A) Ovulation problems
•    Ovulation-stimulating drugs such as:
Clomiphene – Letrozole – FSH stimulants

b) Polycystic ovary syndrome
•    Weight loss
•    Cycle regulation
•    Ovulation stimulation

c) Fallopian tube obstruction
•    The blockage may be surgically opened
•    Or resort to microinjection if the blockage is severe

D) Endometriosis
•    Hormone therapy or laparoscopy to remove cysts and adhesions

E) Poor ovarian reserve
•    Strong stimulation + IVF
•    Or egg donation, depending on the laws of your country

2) Treatment of infertility in men

A) Varicocele
•    Treatment with surgery or laparoscopy if there is an effect on the semen.

B) Poor sperm quality
•    Antioxidant supplements
•    Treatment of infections
•    Improvement of diet and sleep
•    Smoking cessation
•    In severe cases → Intracytoplasmic sperm injection (ICSI)

c) Azoospermia
•    If the cause is obstruction → Surgical treatment
•    If the cause is poor production → Testicular sperm extraction (TESE)

d) Hormonal disorders
    •    Treatment of hormone deficiency or excess based on test results

Third: Assisted fertilization options

1) Intrauterine insemination (IUI)

Suitable for simple cases.

2) In vitro fertilization (IVF)

For blocked fallopian tubes or ovulation problems.

3) Intracytoplasmic sperm injection (ICSI)

The best method when there is a severe problem with the man.

Is treatment expensive?
Treatment can be more expensive than other simpler methods, but not all couples need IVF straight away; doctors always try to choose the most suitable and least expensive methods first, if possible.
Can I exercise during treatment?
You can often continue with light activities, but it is best to avoid strenuous exercise or intense sports during the stimulation period or after embryo transfer, as directed by your doctor.
Could I be eligible for intrauterine insemination (IUI) before IVF?
Yes. In some cases where fertility is not completely impaired, intrauterine insemination (IUI) may be the first treatment option before moving on to IVF if it is unsuccessful.
When can I take a pregnancy test after embryo transfer?
A blood pregnancy test is usually performed about 10-14 days after embryo transfer to confirm pregnancy.
